For the issue of a certificate of competency: $10.00
For the issue of a duplicate certificate of competency: $5.00
For the revalidation of a certificate of competency: $10.00
For the issue of a certificate of endorsement: $18.00
For the issue of a duplicate certificate of endorsement: $18.00
